to report a rare postoperative complication of severe progressive corneal melting and perforation. this happened within 14 days after exposure to mitomycin C and a topical nonsteroidal antiinflammatory agent, each of which has been independently associated with corneal melting. The patient underwent combined phacoemulsification and pterygium excision with mitomycin C application, followed by post-operative usage of a topical non-steroidal anti-inflammatory agent. The management was composed of initial cyanoacrylic glue, followed by tectonic penetrating keratoplasty.
This report is a case report of an emergency referral that occurred at Prince Sultan Medical Military City, at Riyadh, Saudi Arabia
The case report highlights the synergistic effect of both mitomycin C application and topical nonsteroidal antiinflammatory agent in provoking post-operative corneal melting. This might initiate further studies exploring combining both versus using each one separately to avoid the drastic corneal melting.
The use of mitomycin C in pterygium excision, may not be combined with the post-operative application of NSAID. as both my accelerated corneal melting.
