Purpose: To share our short experience, our tips and tricks, in 5 patients with an anterior IOL (Artisan )submited to a Descemet's stripping automated endothelial keratoplasty (DSAEK)
Material: 5 patients with an anterior IOL (Artisan) were submited to DSAEK due to cornal decompensation. 3 of these patients had a posterior cataract luxation and therefore were previously operated on pars plana posterior vitrectomy and Artisan IOL implantation. The other 2 had been submited to a complicated cataract surgery with only an anterior vitrectomy and Artisan implantation.
Method: In patients with a previous posterior vitrectomy, a pars plana infusion was used in order to mantain the anterior chamber depth, and only an anterior chamber mantainer in the other cases. Busin injector was used in 3 patients and a disposable injector (Endoserter) was used in the other 2. Air was used as a tamponade in 3 patients and SF6 gas in the other.
Results: Graft aplication was achieved in every patient although rebubbling was needed in 2 cases. Visual acuity, although limited, was better than in the preoperative evaluation.
Conclusion: Mantainance of the anterior chamber depth was one of the hardest steps of the surgery as well as the tamponade in the postoperative period. We want to share our tips and tricks to make the surgery more uneventful and successful
