To report that acute hydrops can happened as a complication of epi-of CXL in thin corneas

A case of moderate keratoconus with 59K , pachymetry was 395 µ at thinnest location and back elevation was +90 . UCVA was 0.3 and BCVA was o.6. Epi-off CXL was done with accelerated protocol with 9 min time of UV .2nd day postoperative sever central corneal edema developed with corneal pachymetry 754µ with small descemet break, epithelial defect with drop of vision to 1/60 .Medical treatment of acute hydrops in the form of sodium chloride eye drops ,steroids and anti-glaucomatous eye drops were given with improvement of condition over 2 weeks of medical treatment and improvement of vision and returning to the preoperative value after 2 months

Acute hydrops can be triggered by epi-off CXL in thin corneas which in this case respond to medical treatment in short period of time
