To describe the outcomes of the use of the Tenon patch graft in the management of corneal perforation in a peripheral ulcerative keratitis and the monitoring by anterior segment OCT (AS-OCT) to quantitatively and qualitatively assess the effectiveness of the technique.

This study is made in Ophtalmology department at HASSAN II university hospital, Fez. Morocco, SIDI MOHAMED BEN ABDELLAH university Fez Morocco

A 43-year-old women who had been followed for severe dry eyes due to rheumatoid polyarthritis for 20 years, presented to the emergency with a peripheral corneal perforation, sealed by the iris, accompanied by significant corneal edema in the right eye.

A patch of Tenons capsule is harvested from the patient himself in the superonasal quadrant. after the creation of stromal pocket in 360°, around the perforation, the graft is placed over the defect and sutured to the cornea. Using a 30-G needle, the anterior chamber is formed by a small air bubble. 

Before surgery AS-OCT shows that the iris plugs the perforation, the loss of the depth of the anterior chamber. 1 day postoperative AS-OCT shows Hyper-reflective area of the tenon patch graft, masking of posterior structures, with early formation of a corneal scare inside the tenon patch graft, AS-OCT shows the corneal reshaping with continued healing process approaching the healthy corneal area

The AS-OCT evaluates the reformation of the anterior chamber, the individualization of adhesions between the iris and the Tenon capsule when the conditions of non-transparent media do not allow this evaluation, and the healing and reshaping of the cornea. It assess quantitatively and qualitatively the effectiveness of this surgical technique.
