he calculation of the intraocular lens implant is a very important step before cataract surgery, but we could have some postoperative refractive surprises affecting the visual acuity.

 

Should we explant the IOL or the piggyback implantation will be better in this cases?

M ,56 YO

Diabetes (2008) Bilateral Glaucoma (2013)

Monophthalmus

The patient had a cataract surgery 2 months before consulting because of his low vision.

He had 1 year before a detachment retinal surgery (with silicone oil injection).

IOL calculation was done regardless of the presence of Silicone oil, the patient had an IOL power of -2D.

VA without correction = hand motion Refraction = +17 VA with correction +17 = 5/10.

 

fter 1 month: VA with correction +13 = 5/10.

 

We know that: 22D implant           13D glasses

So the IOL power = + 22 D. 

 

A Piggyback implantation in the sulcus with an IOL MA60AC (Alcon) 3 pieces  acrylic hydrophobic was done.

1 week after surgery, VA without correction = 5/10.

 

1st Piggyback implantation was performed at 1999 by Luis Mejia.

José Alfonso publishes in 2006 a series of 6 patients with a good results.

Léonardo Akaishi reports in 2007 the results of a series of 13 eyes from 7 patients, after a series of 20 eyes was done also with a sufficient results.

 

Piggy-back is a surgical technique that has its place in the context of cataract surgery.

The classic indication for correcting postoperative ametropia is supported by the availability of implants specifically adapted to this type of implantation.

The indications using multifocal implants are more recent and are integrated into the therapeutic strategy of surgeons of cataracts with strong refractive tropism.
