To take a case of extreme refractive complications relegated to highly interventional procedures with poor unaided vision expectations, and plan a staged Corneoplastique approach using least interventional, visually- directed techniques, straight to unaided vision freedom.
Single Institution, Single surgeon retrospective case analysis
A 75 year old Nurse was referred with preoperative vision of CF 4 feet with 23.50D Irregular Astigmatism, Keratometry of 88.90 D, status post hexagonal Keratotomy surgery with ectasia and scarred Cornea, High Myopia, Fuchs Dystrophy, Subluxated Cataract, Amblyopia and Exophthalmos.
Using Corneoplastique principles, a staged approach was planned with INTACS surgery that made this cornea “measureable” while decreasing Keratometry and Astigmatism from 23.50D to 1.4D, controlling ectasia and not harming the Fuchs dystrophy. Four months later, on confirming stability, LenzOplastique based Toric premium cataract surgery was performed resulting in unaided vision of 20/25
