A case-report of an eye with traumatic subluxated cataract and zonular dehiscence and intraoperative use of a capsular ring, a capsular segment with scleral fixation and capsular retractors.
Case report
A 78 years old patient with senile nucleocortical cataract of RE, had a blunt trauma in the same eye - hit by a branch of a tree.
Status: VOD= 0.09-0.1, TOD=17 mm Hg. Anterior segment - cornea with senile arcus, iris with atrophic pupillary margin, iridophacodonesis, subluxated, nucleocortical cataract, the equator of the lens can be seen temporally, vitreous prolapse in the pupill temporally.
Capsular retractors were used during surgery, and a capsular tension ring and a capsular segment with scleral fixation (reverse Hoffman pocket) were implanted. Phacoemulsification and anterior vitrectomy were performed with subsequent implantation of a 3-piece monofocal hydrophobic acrylic intraocular lens in the capsular bag.
One month postoperatively VOD=0.7, the IOL in the bag is well-centered.
Capsular segments with scleral fixation are an effective means of addressing the challenge of zonular weakness and dehiscence in cataract surgery, providing an opportunity for both intraoperative capsular support and an approach to center the implanted in the bag IOL postoperatively.
