When The Iol Refuses To Go In

Cataract surgery involves several steps for a successful outcome. One of them is the correct lens implantlation inside the capsular bag. The purpose of this case is to report an unusual situation in which the lens was inadvertently injected into the corneal stroma dissecting the stromal lamellae.

A 74 year-old patient was undergoing phacoemulsification combined with trabeculectomy. After cataract removal, the eye was filled with OVD and the IOL was loaded into the injector. Injection was attempted by using the wound-assisted technique. However, instead of going into the capsular bag, the IOL was injected into the corneal stroma. The operating surgeon realized the misplacement of the lens and removed it from the cornea using a regular pair of forceps. The IOL was then reinjected, this time, inside the capuslar bag. Surgery was concluded with a full air fill in the AC. Patient had a slowly recover corneal oedema resolving few weeks after surgery.
Cataract surgery is a modern surgery that require meticulous management of each of its steps. Surgeons need to be attentive through the entire procedure to avoid unusual complications.
