Cataract Phacoemulsification With Toric Mutifocal Iol Implantation In A Patient With A Previous Photorefractive Keratectomy.

We present the case of a cataract phacoemulsification with Lentis Mplus intraocular lenses (IOLs) implantation in a patient with a previous photorefractive keratectomy (PRK).

PRK was performed more then 20 years ago for reason of optical correction of a moderate myopia. Preoperatively nucleus cataract with corneal astigmatism 2,25-3,0 D was diagnosed in both eyes. Since the primary visual requirements of the patient were for distance and near, with fine details a toric multifocal IOL was chosen for implantation. Lentis Mplus IOLs with individually customized cylinder correction (Lentis Mplus LU-313 MF30T IOL) were implanted consequently, in both eyes after uneventful phacoemulsification. The Haigis-L formula was chosen for IOL power calculation because of previous myopia correction, the manufacturer customized the cylinder correction according to the corneal refractive power.
Emmetropia in both eyes was achieved.Three months postoperatively, the uncorrected distance visual acuity had increased from 0.20 logMAR to 0.10 logMAR in the right eye and from 0.20 logMAR to 0.00 logMAR in the left eye. The patient gained 6 lines of uncorrected near visual acuity: 0.20 logMAR in the right eye and 0.10 logMAR in the left eye. Patient was completely satisfied with his visual functioning.
Customized multifocal toric IOL implantation with calculation IOL power according Haiges-L formula have demonstrated good three months visual results for cataract treatment after PRK.
