To report intraoperative complications arising during activation of CO2 mechanism of Clareon (alcon) IOL.
CO2 mechanism used to deliver the IOL into the capsular bag after phacoemulsification surgery is very effortless. However,rare complications/accidents may occur while injecting the IOL from the injector.Report of 3 cases.
We report very rare complications( 3 cases) which had occured during injection of Clareon (Alcon) IOL .
Case 1: In a posterior polar cataract after uncomplicated phacoemulsification surgery Clareon lens was introduced through the corneal tunnel into the capsular bag(CB). Forceful Pressure was given onto the lever of the injector throughout. The IOL released forcefully into CB and hit the thin posterior capsul (PPC) thus causing a central rent . As the trailing heptic was out side the CB the IOL could be repositioned into the CB without further complications.
Case 2 and case 3 : after breakage of the CO2 cannister(of the injector) pressure was kept onto the lever with the index finger. The surgern was distracted due to sudden coughing by the patients and the IOL were released(from the injector) out of the eye over the cornea. The IOL were re-injected into the capsular bag with the use of seperate cartridge.
Clareon IOL injector system deliver the IOL into the eye smoothly. However, if full pressure is given onto the release lever CO2 gas can expell the IOL forcefully and cause PC rent . Specially ,in case of preexisting capsular thinning(PPC). Accidents do happen but can be managed sucessfully.
