To describe the functional results after implantation of Vivity in patients requiring DSO and cataract surgery in Fuchs Sindrome.

A case of Fuchs Dystrophy and cataract treated with DSO, phacoemulsification and Clareon® Vivity® extended-focus intraocular lens implantation is presented.
Female, 49-year-old patient.
The refraction of the patient is right eye -0.75 -2.25x170º and left eye -2.25 -0.75X170º, achieving a visual acuity (VA) in distance vision of 0.1 and 0.4 LogMAR respectively.
Endothelial microscopy performed in the left eye fails to obtain a count of endothelial cells in the center of the cornea, observing a good peripheral mosaic.
In biomicroscopy, Fuchs' dystrophy was observed with a pattern of confluent central guttas in both eyes and N3 corticonuclear cataract in left eye, with no other significant findings in the anterior segment or in the fundus.
Left eye surgery is considered, in which DSO, phacoemulsification and intraocular lens implantation are indicated.
One month after surgery, the VA with a correction of +0.75 reaches 0.2 LogMAR, with fuzziness and little photophobia, the central edema has disappeared, and there are still some folds in Descemet's membrane (MD). After three months, the VA without correction is 0.3 LogMAR and with its graduation: +0.50 -0.50x165º it reaches 0.1 LogMAR, in near vision with an addition of +1.50 the VA is 0.0 LogMAR. One year after surgery, VA without correction has improved to 0.2 LogMAR and with its refraction: +1.00 -0.50x160º it remains stable at 0.0 LogMAR. A repopulation of central endothelial cells is observed.
Non-diffractive Premium platforms may be an option in patients with endothelial pathology and recovery after standardized DSO
