Enhanced Monofocal Iols: A Comparative Study Between Three Different Models

Purpose
Cataract surgery with intraocular lens (IOL) implantation is one of the most frequent surgical procedures performed worldwide. IOLs are continuously evolving in their design, trying to achieve more spectacle independence combined with few visual disturbances. Enhanced monofocal IOLs aim to achieve excellent degree of best distance visual acuity providing some degree of intermediate vision with no visual side effects or loss of contrast sensitivity.
The purpose of the present work was to compare the visual performance and optical quality between three new enhanced monofocal IOLs after cataract surgery.

Methods
This retrospective single-center study included patients affected by cataract with corneal astigmatism less than 0.75 D and no ocular comorbidities undergoing cataract surgery with bilateral implantation of Tecnis Eyhance ICB00 (Johnson & Johnson Vision Care, Inc.), Vivinex Impress (Hoya Surgical Optics, Singapore) or IsoPure (PhysIOL, Liege, Belgium) IOLs. Three months postoperatively, monocular and binocular uncorrected and corrected distance, intermediate and near visual acuities were measured. Binocular defocus curve, photopic contrast sensitivity, Point Spread Function (PSF), low order aberrations (LOAs), high order aberrations (HOAs), objective scatter index (OSI), halo and glare perception were also investigated.
Results
This study included a total of 72 eyes of 36 patients. Monocular and binocular uncorrected and corrected distance, intermediate and near visual acuities were similar between groups. Aberrometric study showed similar results between the 3 groups regarding PSF, LOAs, HOAs and OSI. There were no statistically significant differences between the 3 groups in terms of photopic contrast sensitivity, halo and glare perception.
Conclusions
In patients without ocular comorbidities, the Eyhance ICB00 IOL, the Vivinex Impress XY1-EM IOL and the Isopure 123 IOL, even though based on different optical properties, provided similar results in terms of UDVA, UIVA, UNVA, contrast sensitivity and intraocular aberrations with no influence on photic phenomena.
