Femtosecond Laser-Assisted Mini-Capsulotomy For Intumescent White Cataracts

Purpose
To investigate a new two-step femtosecond laser-assisted technique for intumescent white cataracts.
Setting
Prospective Trial, University Eye Hospital Bochum, Germany
Methods
A prospective trial with 50 eyes of 48 patients with intumescent white cataracts was conducted. After docking with the fluid-filled patient interface, the anterior segment of the eye was measured automatically by the integrated three-dimensional spectral-domain optical coherence tomography system (Catalys Precision Laser System). A mini-capsulotomy of 2 mm was performed to release the intralenticular pressure. Then the lens material was removed from the anterior chamber with irrigation and aspiration, and a larger, regular capsulotomy was performed manually or with the femtosecond laser under sterile conditions.
Results
The anterior lens capsule could be visualized in all cases and identified for treatment. While performing the capsulotomy, an explosive discharge of lens material could be observed regularly. The larger, second capsulotomy could be performed in 49/50 cases without complications. In one case a radial tear occurred due to an application error during the second larger femtosecond laser-assisted capsulotomy.
Conclusions
Femtosecond laser-assisted mini-capsulotomy offers a safe way to perform capsulotomy and has the potential to avoid complications in intumescent, white cataracts.
