Purpose

This report describes a case of lenticular coloboma without cataract and anisometropia managed by Implantable Collamer Lens (ICL) implantation and complicated with ICL subluxation 3 weeks postoperatively.

Setting

Teretiary eye center

Report of case

27-year-old gentleman presented to King Khaled Eye Specialist Hospital (KKESH) eager to get refractive surgery. Slit lamp examination revealed  inferior lens coloboma. Given the presumed small area of lens coloboma and absence of cataract, Toric-ICL implantation was performed after explaining risks and benefits. The implanted ICL was placed away from the lens coloboma and on the intended axis as provided by the manufacturer. Three weeks postoperatively the ICL was seen inferiorly subluxated and the ICL rotation became more than 30 degrees. The ICL was explanted safely from the eye and postoperative period was unremarkable.

Conclusion / Take home message

Implantation of ICL requires 360-degree lenticular support to ensure stability of the implanted lens. Additionally, full pupil dilation is a crucial during assessment of lenticular coloboma to define the maximal extent of the defect.
