Purpose
To measure binocular vision in refractive cataract patients implanted with various increased or full range of vision implants immediately postoperatively between 1-2 hours.
Setting
Single surgeon single center and single surgical ambulatory surgical center study of 1172 eyes of 586 patients undergoing routine cataract surgery.
Methods
Retrospective chart review, insignificant risk, comparative study from a single surgeon and single center. Patient eyes (N-1172) fell in to 1 of 5 groups. Those receiving extended range of focus lenses: (Rayner EMV, Eyhance, or Vivity) or small aperature lenses (AcuFocus) or those receiving full range of focus lenses (Synergy or Panoptix). One-to-two-hour vision testing was done with the Lighthouse ETDRS charts for distance and near. Patients had surgery using the femtosecond laser and standard phacoemulsification with implantation of either toric or non-toric lens. The eyes were treated sequentially (one to two week). Lens powers used ranged from 5 to 32D with toricity to 6D. Many extended range of focus lenses were defocused for near.
Results
Over 90% of patients have near vision J1 or better. Defocus in the extend range of focus lenses is necessary (contact lens trials preoperatively work). The full range of focus lens achieve the best near vision with 100% J1 or better but distance vison better than or equal to 6/6 was achieved more frequently in the extended range of focus lenses. Many patient had previous refractive surgery and enjoy similar immediate vision once the second eye had been treated. The enhancement rate in this group was 1.37%.
Conclusion
With proper planning, preoperative surgical diagnostics and treatment, intraoperative diagnostics, and counseling patients can receive great immediate outcomes. Many patient’s had previous refractive surgery and note similar immediate vision once the second eye has been treated. Low enhancement rates were seen.
