Purpose

To establish the effect of the capsulorhexis diameter on the posterior capsule opacification (PCO) development after the implantation of three different acrylic intraocular lenses (IOLs).

Setting

The study was conducted in Clinic of ophthalmology, University clinical centre Kragujevac, Serbia.

Methods

The study included 93 patients with senile cataract scheduled for phacoemulsification. It was designed as a prospective, clinical study from January 2020 until January 2022.  According to the IOL type the patients were randomized into three groups: I group: hydrophilic acrylic, single-piece IOL (Eyecryl plus 600, Biotech visioncare); II group: hydrophobic acrylic, single-piece IOL (AcrySof SA60AT, Alcon laboratories); III group: hydrophobic acrylic, three-piece IOL (AcrySof MA60AC, Alcon laboratories). Every group was further divided into two subgroups according to the capsulorhexis diameter (I subgroup > 5mm, II group <5mm). Posterior capsule opacification was measured 1, 6, 12, 18 and 24 months after phacoemulsification using EPCO software.

Results

During the first postoperative year no statistically significant difference in PCO development  was noticed in all groups depending on the capsulorhexis diameter (p>0.05). Eighteen and twenty four months after phacoemulsification patients from the first and second group who had capsulorhexis diameter lower than 5mm had statistically significantly lower PCO compared with patients from the same groups, but with capsulorhexis diameter over 5mm (p<0.05). This trend was continued to end of the study in these groups, while during the whole research capsulorhexis diameter had no influence on the PCO development in patients with AcrySof MA60AC IOL.

Conclusion

The study has shown that capsulorhexis diameter had a huge effect in PCO development in patients with single-piece IOLs, while in patients with three-piece IOLs capsulorhexis diameter had no effect during two years of postoperative period.
