Purpose

To investigate changes in the cornea after therapeutic fresh myopic lenticular implantation in advanced keratoconus with femtosecond laser-assisted small incision (SMILE module) surgery.

Setting

Eye Hospital,Pristina,Kosovo

Methods

Forty eyes with advanced keratoconus indicated for corneal transplantation were included in this study. Fresh myopic lenticular implants were placed in all eyes through SMILE module surgery. Lenticular implants were extracted from patients with myopic refractive errors of the cornea. Using the VisuMax femtosecond laser with the medium cone and 130-nJ energy, we created a stromal pocket with a diameter of 8 mm (larger than the optical zone of the donor lenticule) and cap thickness set to 130 µm from the corneal surface and a 2.09-mm incision. The lenticule was held with the lenticule forceps and gently inserted into the pocket through the 2.09-mm incision. The incision position changed according to the position of the highest  Keratometric values.

Results

In the 1-year group, the preoperative CCT increased from 338.35 ± 11.24 µm (range: 300 to 349 µm) to 474.35 ± 4.31 µm (range: 469 to 482 µm) (P < .001). In the 2-year group, the preoperative CCT increased from 365 ± 6.91 µm (range: 353 to 377 µm) to 481.95 ± 6.00 µm (range: 473 to 491 µm) (P < .001).The preoperative CDVA improvement from 1.47 ± 0.31 logMAR (range: 1 to 2 logMAR) to 0.48 ± 0.06 logMAR (range: 0.40 to 0.52 logMAR) (P < .001) in the 1-year group . The pre-operative CDVA improvement from 1 ± 0 logMAR (range: 1 to 1 logMAR] to 0.55 ± 0.09 logMAR (range: 0.40 to 0.70 logMAR) (P < .001) in the 2-year group .

Conclusion

Trepatic intrastromal lenticular keratoplasty, not only does the corneal central thickness and visual acuity increase but also provides advanced cornea of keratoconus regeneration because it contains fresh lenticular stem cells and telocyt.
