Purpose

To describe the management of an interesting surgical case in patient with a severely distorted anterior chamber.

Setting

Tertiary Centre Operating Theatre

Methods

55 year old lady presented to hospital with a large anterior staphyloma secondary to a perforated bacterial corneal ulcer. Her vision was no perception of light and she had significant conjunctivalisation of the cornea with protrusion of the staphyloma. An US B-scan showed a large distance of 8.9mm between the posterior surface of the cornea and the anterior lens, a dense cataract and no evidence of retinal or choroidal detachment.

Results

We performed a 12mm limbus to limbus penetrating keratoplasty with combined cataract extraction in order in order to restore some visual potential and to improve cosmesis.

Conclusion

We have shown that in cases with severely distorted anterior chamber architecture, successful outcomes can be achieved with large diameter penetrating keratoplasty.
