Purpose

The aim of this study was to investigate the effects of trifocal and monofocal intraocular lens with enhanced depth of focus design implantations

Setting

Retrospective non-randomized clinical trial

Methods

40 patients who had bilateral implantation of the trifocal (VSY, AcrivaUD Trinova Pro C) or monofocal extended depth of focus design (Johnson&Johnson Vision Care, Tecnis Eyhance) intaocular lenses after phacoemulsification were included in the study. The preoperative and postoperative examination findings of the patients were analyzed. At the 6th postoperative month, binocular defocus curves, contrast sensitivity measurements, subjective complaints, spectacles independence, and the quality of life questionnaire results of the VF-14 questionnaire without spectacles and with spectacles, and satisfaction assessments were also examined.

Results

The mean age of the subjects participating in the study was 61.08±9.55 years. At 6 months postoperatively, spherical equivalent values were -1.06±0.36 D in the Trinova Pro  group, -0.14±0.36 D in the Tecnis Eyhance group, and the difference was statistically significant. While distance and intermediate visual acuities were similar at 6 months postoperatively, monocular and binocular near visual acuities were found to be statistically significantly better in the Trinova Pro group. The contrast sensitivity values obtained at frequencies of 12 cpd and 18 cpd in photopic without glare and 12 cpd in photopic with glare measurements were found to be statistically better in the Tecnis Eyhance group. In the Trinova Pro group, 25% of the patients had low-intensity photopic complaints and 5% of the patients had moderate intensity of photopic complaints respectively, while 10% of the patients in the Tecnis Eyhance group had low-intensity photopic complaints, and the difference between the two groups was statistically significant. While spectacles independence could be achieved in all patients in the Trinova Pro group, in the Tecnis Eyhance group, 60% of patients always, 15% of patients often, and 5% of patients occasionally needed spectacles. The results of the VF-14 surveys showed that the Trinova Pro performed significantly better in the survey results without spectacles. Finally, 95% of Trinova Pro group patients and 90% of Tecnis Eyhance group patients reported that they could recommend the implanted intraocular lens to people around them.

Conclusion

Satisfactory visual results were achieved following the implantation of the new generation diffractive trifocal intraocular lens Trinova Pro and the extended depth of focus design intraocular lens Tecnis Eyhance. Trinova Pro performed better at near, although far and intermediate vision was comparable between the groups. On the other hand, a higher rate of photic phenomena was observed in the Trinova Pro group. Patient satisfaction was convincing for both groups of intraocular lenses. More clinical studies are needed to examine the visual quality, contrast sensitivity values and photic complaints of these iols.
