Purpose

we are willing to describe a bevel-down phaco technique using an IOL to protect the posterior capsule from the vacuum and the endothelium from the ultrasounds 

Setting

phaco-emulsification of a hard nucleus 

Methods

injecting the IOL before totally removing the nucleus to permit working deeper in the anterior chamber far from the endothelium and avoiding the aspiration of the posterior capsule 

Results

we can easily use more vacuum and more ultrasound

Conclusion

using an IOL befor ending the phaco-emulsification of totally of the nucleus is a safer technique for the endothelium and the posterior capsule
