Purpose
This study aimed to evaluate the corneal endothelial protective effects of four commonly-used OVDs formulas during non-complicated phacoemulsification.
Setting
A single-center, prospective, randomized non-inferiority trial.
Methods
Fifty-two patients diagnosed with cataracts without other underlying ophthalmic pathologies, particularly glaucoma and retinal disease, were enrolled in the study. Seventy-two eyes were randomly assigned into four groups according to different OVDs used during Phacoemulsification, including Healon®, Healon GV®, IAL-F®, and Visiol®. Endothelial cell count (ECC) was measured by non-contact specular microscopy (TOMEY Corporation, Nagoya, Japan) pre-operatively and at 1 day, 1 week, 1 month, 3 months, and 6 months post-operatively.
Results
Overall, 72 eyes of 52 patients were randomized into 4 groups; there was no significant difference in corneal endothelial cell density (ECD) at baseline between groups. Additionally, the ECD at all post-operative follow-up visits in 4 different OVD groups were not statistically significant (All P >0.05). The mean (standard deviation, SD) endothelial cell loss at 6 months post-operative were 5.75% (8.96%), 11.22% (11.99%), 12.76% (16.94%), and 8.59% (10.47%) in the Healon®, Healon GV®, IAL-F®, and Visiol® groups, respectively (P=0.368). Though, Healon® and Visiol® were likely to have superior protective effects on endothelial cells during phacoemulsification, but not statistically significant. Notably, all cataract surgeries were performed with no complications by a single surgeon. Overall mean (SD) surgical duration was 7.82 (2.1) minutes.
Conclusion
Corneal endothelial damage during phacoemulsification is one of the major concerns for all ophthalmologists. Several OVDs have been developed to improve the protective effects on endothelial cells during the procedure. From this study, four different OVDs did not show significantly different endothelial protective effects, in terms of endothelial cell loss, during the non-complicated phacoemulsification by an experienced surgeon.
