Purpose
To present the case in which cataract surgery without IOL implantation was performed the day before in another healthcare institution
Setting
University eye Clinic, KBC Zvezdara, Belgrade, Serbia. The operation was performed in October 2022
Methods
On admission visual acuity was L+/P+, IOP was 34 mmHg. Preoperative examination revealed corneal edema due to the elevated IOP, as well as anterior chamber filled with swollen cortical masses. 23G trocars for PPV were placed. Lavage of anterior chamber was performed with vitrectome probe and viscoelastic (to prevent endothelial damage) so the view of anterior parts of vitreous is obtained. Complete vitrectomy was performed with phacofragmentation of the lens nucleus and cortical masses. After that capsular bag was deliberated using vitreous cutter and intact anterior capsule was found, so we decided to implant three piece IOL in the ciliary sulcus. After the hydration of main incision and paracentesis openings, trocars were removed
Results
The first postoperative day VA was 0,2 Snellen (corneal epithelial edema was present). OCT of the macula was normal
Conclusion
PPV with secondary IOL implantation is a safe and effective procedure that results in a significant improvement in postoperative visual acuity if a technique is adequate and well chosen to the individual case.
