Purpose
To present a challenging case of complicated cataract surgery with no capsular support in which four flanged scleral fixation technique was practiced with a two haptic foldable spherical hydrophylic IOL.
Setting

Methods
We present a case of scleral fixation after complicated cataract surgery, with no capsular support. He is a 74 years old patient who underwent a complicated cataract surgery 1 week before. Cornea was clear and no cortex or nucleus drop during first surgery. We went with a four flanged technique scleral fixation of the foldable IOL using a 7.0 polypropylene suture. The results were very good and we had a stable and centered IOL in the next month after surgery with the BCVA 18/20.
Results
Scleral fixation with the four flanged technique using a two haptic foldable spherical hydrophilic IOL in this complicated case of cataract, was successful and the patient has BCVA 18/20 the next month after the operation.
Conclusion
Using a two haptic foldable IOL is not preferred in scleral fixation technique due to the difficulty in maintaining the perfect plane and stability, but in this challenging case it is done with very good results.
