Purpose

To report effectiveness of recombinant human tissue plasminogen activator (alteplase) in severe post operative fibrinous reaction.

Setting

University Hospital Center of Tirana “Mother Teresa”

Department of Ophthalmology

University of Medecine, Tirana

Methods

We present and review a series of cases that were treated with intracameral recombinant tissue plasminogen activator (r-tpa), after formation of persistent inflammatory membrane post uneventful cataract surgery. The patients were injected a single dose of r-tpa, intracameral, after failing with anti-inflammatory drugs. The outcome was positive after 24h of intracameral injection despite the others patient’s underline health problems. R-tpa was injected on day 5-12 after surgery and caused fibrinolysis in all the presented cases.

Results

In all patients with persistent inflammatory membranes who were applied intracameral alteplase, fibrinolysis of the membrane occurred and they had excellent results.

Conclusion

Intracameral injection of recombinant human tissue plasminogen activator (alteplase) has a high effectiveness in fibrinolysis of persistent inflammatory membranes post cataract surgery.
