Purpose
To compare the Dynamic Corneal response (DCR) parameters obtained with the Corvis ST at baseline (treatment naïve patients) and one month after initiating monotherapy with prostaglandin analogues (PGs) in eyes showing visual field (VF) progression or stability after six years follow-up.
Setting
Hospital Universitario Príncipe de Asturias, Universidad de Alcalá, Alcalá de Henares, Madrid, Spain.
Methods
This is a prospective observational study. Newly diagnosed and treatment naïve OAG patients were examined at baseline, and one month after beginning monotherapy with topical PGs. Goldmann applanation pressure (GAT IOP) and the corneal Hysteresis (CH), and the DCR parameters were measured using the ORA and the Corvis ST devices, respectively. Then the VF defect progression was evaluated 6 years after baseline visit.
Results
A total of 63 eyes were analyzed. 47 eyes were considered as “non progressed” (NP) and 16 eyes were considered “progressed” (P) according to the “event analysis” evaluation of the VF performed by the Humphrey VF analyzer after 6 years follow up. There were no significant differences in baseline parameters between NP and P groups. Nevertheless, in the 1 month follow up exam, the SSI was found to be 1.60±0.34 vs 1.90±0.34 (p=0.003) and the CH (mmHg) was 10.26±1.73 vs 9.34±1.59 in the NP vs P groups respectively (p=0.06). No significant difference was found in any other parameter analyzed.
Conclusion
High speed corneal imaging of the deformation induced by an air puff (Corvis ST) provides a corneal rigidity index (SSI) that shows a high correlation with VF progression when measured 1 month after initiating PGs therapy in newly diagnosed and treatment naïve open angle glaucoma.
