Purpose

In this study, it was aimed to evaluate corneal biomechanics in patients with cystinosis and to compare with healthy patients.

Setting

Patients with cystinosis and healthy subjects were included in this prospective study.

Methods

A detailed ophthalmological examination was performed in all cases. Corneal biomechanical parameters were evaluated with Corvis ST® (Oculus, Wetzlar, Germany). The data obtained from the two groups were compared and analyzed statistically.

Results

Twenty eyes of 10 patients with cystinosis (5 females, 5 males) with a mean age of 17.2±7.9 years and 20 eyes of 10 healthy individuals (6 females, 4 males) with a mean age of 12.5±4.6 years were included in the study. There was no significant difference between the two groups in terms of age and gender (p=0.126, p=0.35, respectively). In the patient group, cystine crystals were deposited in the central and peripheral cornea in all cases.

The mean intraocular pressure (IOP) values obtained by Goldmann applanation tonometry, noncontact tonometry, and Corvis ST® in patients with cystinosis were 14±1.4, 16.6±1.8, and 16.2±2 mmHg, respectively. IOP values obtained by three different methods were found to be similar (p=0.152). First applanation velocity (A1V), deformation amplitude (DA), DA ratio (DAR) and integrated radius (IR) were found to be statistically significantly higher in patients with cystinosis compared to the control group (p=0.001, p<0.001, p=0.004, p<0.001, respectively). On the other hand, the second applanation velocity (A2V), first applanation stiffness parameter (SP-A1), central corneal thickness (CCT), stress-strain index (SSI), and peak distance (PD) at the highest concavity point were found to be low in patients with cystinosis (p=0.001, p<0.001, p=0.003, p=0.002, p=0.003, respectively).

While there was a negative correlation between age and the PD at the highest concavity point (r= -0.336, p=0.034), no significant correlation was found with other parameters (p>0.05 for all).

Conclusion

In this study, it was concluded that the cornea was less resistant to deformation and biomechanically weaker in cystinosis patients compared to healthy individuals. Moreover, it has been shown that the cornea may be less resistant to deformation with increasing age and prolonged disease duration in patients with cystinosis.
