Purpose

To show the usefulness of anterior segment OCT (AS-OCT) in the diagnosis, personalized treatment and follow-up of patients with different types of glaucoma.

Setting

We present three patients with three different types of glaucoma. All of them were examined with AS-OCT (Casia 2) at the first appointment to evaluate the structures of the anterior chamber and plan a personalized surgical treatment. At follow-up, AS-OCT showed the results in the anterior chamber after surgery.

Methods

Three patients with glaucoma were examined at the first appointment with the slit lamp and AS-OCT. The AS-OCT helped us in the diagnosis and decision of the best surgery to perform in each patient. In the follow-up, all patients were also scanned with AS-OCT to see the results of the surgery and the structures in the anterior chamber.

Results

The first patient had a lens luxation in the anterior chamber due to trauma. We performed an extraction of the dislocated lens and an anterior vitrectomy, leaving the patient in aphakia, requiring secondary IOL implantation and glaucoma surgery.

The second patient had high myopia, corrected with an Artisan lens. After performing a trabeculectomy, we observed a post-surgical athalamia in the AS-OCT that improved with medical treatment.

The third patient was also high myopic corrected with Artisan lens, who presented a pupillary block. AS-OCT showed a pupillary block with anteriorization of all the structures of the anterior chamber. It was resolved with a surgical iridectomy.

Conclusion

AS-OCT can be useful for evaluating the affected structures in the anterior chamber of patients with glaucoma, knowing the etiology and helping to decide which surgical tecnique is the most appropriate in each case. In addition, it is useful in the follow-up of these patients to assess the final result of the surgery.
