Purpose

Posner-Schlossman syndrome is a recurrent unilateral glaucomaticocyclitis crisis.

The diagnosis is difficult and even misleading.

It is the stereotypy and the chronology of the symptoms that guide the diagnosis.

The purpose of our work is to demonstrated the role of the glaucogram in the diagnosis of this syndrome.

Setting

We are going to report the observation of a patient followed in our training (period of 18 months) for unilateral glaucoma and whose glaucogram allowed us to establish the diagnosis.

Methods

It is a 45-year-old patient (with no particular history) who presented 3 separate attacks of unilateral intraocular hypertension in the right eye (46 to 48 mmHg) (open angle on gonioscopy).

A dual therapy was prescribed for the right eye. The follow-up showed a normalization of the IOP (between attacks) with a normal assessment (optical coherence tomography, visual field).

The last 2 attacks were associated with a Tyndall between trace and 1+.

The glaucogram showed a stereotypical profile of the IOP curve with a characteristic “sawtooth” aspect. This made it easier for us to diagnose the Posner-Schlossman syndrome.

Results

Posner-Schlossman syndrome is a rare condition (corresponds to 4% to 5% of the causes of hypertensive uveitis).

It has been incriminated by several authors as an inflammatory reaction secondary to Cytomegalovirus replication.

Characterized by the occurrence of unilateral recurrent attacks of acute hypertensive anterior uveitis, with little inflammation.

The attacks are limited in time and usually resolve spontaneously, regardless of the treatment instituted.

Conclusion

Posner-Schlossman syndrome is difficult to diagnose.

The glaucogram is a clinical follow-up tool that allows diagnostic revision. It makes it possible to orient the latter by showing the characteristic “sawtooth” aspect of the IOP curve.
