Purpose

Selective laser trabeculoplasty (SLT) is often the first-line of treatment for glaucoma patients. However, its impact on the intraocular pressure (IOP) of the untreated fellow eye remains unknown. Therefore, we conducted this meta-analysis to determine such changed following SLT in the glaucomatous eye.

Setting

This is a systematic review and metaanalysis studying the effect of selective laser trabeculoplasty on the intraocular pressure of the fellow untreated eye.

Methods

The protocol is registered on PROSPERO (CRD42022364621). On September 19, 2022, nine databases were searched. We included articles that reported the change in IOP of the fellow untreated eye following SLT. All analyses were conducted using STATA. The fixed-effect model was used due to the absence of heterogeneity, while the inverse-variance method was used to pool the weighted mean difference (WMD) and its corresponding 95% confidence interval (CI) across studies. Subgroup analyses were conducted based on the assessment timepoints.

Results

Five studies (183 eyes) were analyzed. Compared to baseline values, there was a significant reduction in the IOP of the fellow untreated eye [MD= -1.93; 95%CI: -1.50: -2.35, I2=0%]. This reduction in IOP was consistent after 1 week [MD= -1.47; 95%CI: -0.56: -2.39, I2=0%], 1 month [MD= -1.95; 95%CI: -1.13: -2.76, I2=0%], 3 months [MD= -1.52; 95%CI: -1.06: -1.97, I2=0%], and 6 months [MD= -1.99; 95%CI: -1.57: -2.41, I2=0%] of SLT. The pooled WMD estimated a mean reduction in IOP of 2.13 mmHg [95%CI: 1.40: 2.85].

Conclusion

Our findings highlight a protective effect of selective laser trabeculoplasty on the intraocular pressure of the fellow untreated eye. These findings are yet to be confirmed in large-scale randomized trials.
