Purpose
to evaluate corneal astigmatism pattern as predictive biomarker for efficacy of penetrating femtosecond laser-assisted limbal relaxing incisions (FS-LRIs), in management of astigmatism, during femtosecond laser-assisted cataract surgery (FLACS).
Setting
Methods
this prospective study enrolled patients, with clinically significant astigmatism, planned for FLACs. The enrolled patients underwent FLACS with FS-LRIs. We measured topographic keratometric parameters preoperatively, at 1 month and 3 months at least. Patients were divided according to topographic pattern of astigmatism in 10 groups: round (A), oval (B), superior steepening (C), inferior steepening (D), irregular (E), symmetric bowtie (F), symmetric bowtie with skewed radial axis (G), asymmetric bowtie with inferior steepening (H), asymmetric bowtie with superior steepening (I), asymmetric bowtie with skewed radial axes (J). Postoperative residual topographic astigmatism was divided based on diopters (D) in categories: 0-0,5 D (1), 0,6-1 D (2), > 1 D (3) and we calculated odds ratios of achieving astigmatism between 0-0,5 D. Further analysis has been performed for preoperatory astigmatism ≤2 D. The vector analysis of surgically induced corneal astigmatism was performed for all treatments.
Results
the study included 50 eyes of 48 patients. The preoperative proportions of astigmatism pattern were A: 5,7% (2); C: 7,6% (4); D: 9,6% (5); E: 1,9% (1); F: 30% (16); G: 3,8% (2); H: 11,5% (5); I 28,8% (15). The symmetrical bowtie astigmatism group (F) had a statistically significant odds ratio (12,76 p 0,0004*) of achieving an astigmatism between 0-0,50 D. There were not significant results for the other patterns. In the subgroup including preoperatory astigmatism ≤ 2 D the odds ratio was (24,20 p 0,0005*). Overall, the mean target induced astigmatism (TIA) was 1,48 ± 0,75 D (range 0,7 - 4,4), surgically induced astigmatism (SIA) was 0,8 ± 0,37 D (range 0,25 - 1,6), the mean difference vector was 1,54 ± 0,75 D and the correction index was 0,63 ± 0,37.
