Purpose

Total (TT) and anterior (AT) central corneal topography of IOLMaster 700 to tomographic maps from Pentacam AXL were compared regarding irregularities, thus the decision for implanting premium intraocular lenses (IOLs).

Setting

Department of Ophthalmology, Goethe University, Frankfurt, Germany.

Methods

In this prospective study healthy eyes (H), irregular corneas (I) and eyes with previous corneal refractive surgery (P) were randomly selected. Five observers (experienced surgeon (ES), surgeon (S), experienced resident (ER), young resident (YR) and IOL calculating optician (CO)) compared the AT and TT maps individually and in comparison of IOL Master 700 and Pentacam AXL, and answered a questionnaire for each image and eye and the compared images. The questionnaire included the similarity regarding regularity of the cornea, the classification of the pictures regarding the included patient groups and the decision of the IOL implantation. Individual answers and agreement of the observers were evaluated.

Results

The study included 25 eyes per group (75 eyes). The correct category (H, I or P) was reported in 64.3% to 79.3% of individual cases. Except one observer (p < 0.001) there were no significant difference between IOL Master and Pentacam image rating (p < 0.05). The observers did not rate a difference between TT and AT (p < 0.05). Most observers said that the irregular corneas are significant different in regularity than healthy or post refractive surgery eyes (p < 0.001). There were significant inter-observer agreements among the five observers for rating implantation premium IOLs 0.137 to 0.374 (p < 0.05) and rating the correct category (H, I or P) 0.453 (p = 0.000).

Conclusion

Compared TT and AT there was no difference in rating of the five observers. Most observers rated the images of IOL Master 700 and Pentacam AXL equally even if irregular corneas were rated as significant different regarding regularity than healthy or post refractive surgery eyes.
