Purpose
To compare the defocus curves of a non-diffractive enhanced monofocal intraocular lens (IOL) with extended depth of focus (EDOF) at different spectacle-induced mini-monovision levels and emmetropia.
Setting
Methods
This study evaluated 36 eyes of 18 patients who were implanted with bilateral Tecnis Eyhance IOL (Johnson & Johnson Vision Care, Inc.) at least 3 months ago prior to measurement date. One emmetropic and two different spectacle-induced bilateral defocus curves were obtained by Multifocal Lens Analyzer (Q Vision) and those three different setups were compared with each other. In setup A, both eyes were emmetrope. Setup B and C were obtained by inducing the refraction of the non-dominant eye by -0.50 D (Setup B) and -1.00 D (Setup C). The area under the defocus curve and above the 0.3 logMAR line was called the area under the curve (AUC). This area was evaluated at different distances as total (AUC-T), far (AUC-F), intermediate (AUC-I) and near (AUC-N). Patients with visual acuity below 0.8 Snellen for any reason were excluded from the study.
Results
AUC-T=1.89±0.59, 2.04±0.60, 2.35±0.19 for setups A,B,C, respectively (p=0.137, 0.004, respectively). There was no significant difference between the AUC-F of all three setups(p=1.00, 0.317, respectively). While there was a significant increase in setups B and C at intermediate distance(AUC-I) (p=0.001,<0.001, respectively), the only significant increase in near vision(AUC-N) was only in setup C (p<0.001).
Conclusion
Our study revealed that when the non-dominant eye refraction of Tecnis Eyhance Monofocal EDOF IOL is -1.00D, an increase in near vision of more than 2 times is observed without affecting binocular distance vision. When the non-dominant eye refraction is -0.50D, intermediate vision is increased but there was no significant increase in near vision. The visual acuity of the patient at near vision was increased by increasing the depth of focus with -1.00D monovision in the non-dominant eye(setup C).
