Purpose

The extended depth-of-focus (EDOF) AcrySof® IQ Vivity® (Alcon Inc., USA) intraocular lens (IOL) technology is a promising one for cataract surgery. The aim of this research was to evaluate functional and visual outcomes after 12 months in a real-world setting.

Setting

Interventional Prospective Monocentric Study

Methods

This is interventional, prospective, monocentric study. We recruited patients who required cataract surgery and implanted the AcrySof® IQ Vivity® IOL. At baseline, a comprehensive ophthalmologic evaluation including best-corrected visual acuity (BCVA) and contrast sensitivity was performed. At one month, six month and twelve months after surgery, we evaluated the refractive outcomes, distant, intermediate, and close vision, contrast sensitivity (using Pelli-Robson Charts), as well as the Quality of Vision score.

Results

A total of 58 eyes were enrolled, with a mean patients’ age of 67.5±5.4 years. There were no intraoperative or postoperative complications. 16 eyes needed toric IOLs. The best-corrected visual acuity increased from 0.38±0.41 LogMAR to 0.0±0.0 LogMAR (p=0.0001). For both distant and intermediate vision, the refractive outcomes were excellent. In 88% of cases, for close vision, a spherical addition of +1.0D was needed, while the remaining 12% needed a +1.50D spherical addition. A significant increase in contrast sensitivity was reported (p=0.0001). The mean score for Quality of Vision was 16.1±5.7. Haloes and glares were the most often reported shortfalls, but only 6% of patient referred significant disturbances in the evening hours. Moreover, the discomfort of those patients had a strong correlation with scotopic pupillometry results.

Conclusion

The AcrySof® IQ Vivity® IOL appeared an effective and safe option for far- and intermediate-vision correction, with a small sperichal addition needed to improve near vision. Furthermore, the rate of visual discomfort is low and limited to particular pupillometry conditions.
