Purpose

Creating a staged procedure to turn a patient with herpetic scar into a candidate for cataract surgery and bringing them to emmetropia without herpetic recurrence.

Setting

Gulani Vision Institute, Jacksonville, FL

Report of case

72 year old Caucasian male was referred with high irregular astigmatism and a history of herpetic scar in the right eye.  At initial visit, he has -1.75D of irregular astigmatism and 20/60 visual acuity at distance and 20/100 at near.  A staged approach was planned to first address the corneal scar and make the eye measurable for cataract surgery.  Lazrplastique ™ was performed and 7 months later, he underwent cataract surgery with a toric lens implant.  He returned with 20/20 best corrected vision at distance OD and 20/25 best corrected vision at near OD without recurrence of herpes through this time period.

Conclusion / Take home message

By stabilizing his cornea and removing the scar first, he was able to undergo precise cataract surgery to perfect his visual acuity and scar. Separating and combining procedures can make patients previously not candidates for cataract into candidates and eventually bring them to emmetropia.
