Purpose
To compare the safety and efficacy of intracameral injection tropicamide+phenylephrine+lidocaine versus topical tropicamide+phenylephrine. To evaluate if intracameral injection has positive influence on the pupil size which could minimize complications due to small pupil during phacoemulsification.
Setting/Venue
Vinayaka mission kirupananda variyar medical college and hospital, salem, tamilnadu, India
Methods
In this comparative study 100 eyes of 50 patients with bilateral senile cataract who underwent phacoemulsification were studied. each patient's eyes were randomly assigned to two groups. One eye was operated after instillation of topical mydriatic (1 drop every 10 mins for 40 mins). The fellow eye was operated only with intracameral injection of mydriatic and anaesthetic (injected after entering into the anterior chamber). Pupil size is assessed during capsulorhexis, hydro dissection, phacoemulsification, cortex wash and IOL implantation. Blood pressure and pulse rate were also monitored.
Results
Patients with topical mydriatics had better dilatation than intracameral mydriatics but pupil size was consistent through out the procedure with intracameral injection. Surgeons comfort was better with intracameral injection. Intra and post operative complications were significantly less in intracameral group.
Conclusions
Intracameral injection of mydriatics and anaesthetic is safe as it has no effect on the blood pressure, pulse rate and it is effective in phacoemulsification as it has significantly reduced the complications due to small pupil. it is extremely benificial in patients undergoing toric IOL implantation
