Clinical Outcomes Of Cataract Surgery In Patients With Sjögren Syndrome

Purpose

To evaluate the clinical outcomes of cataract extraction with phacoemulsification and posterior chamber intraocular lens implantation in patients with Sjögren syndrome (SS) compared with non-SS dry eye.

Setting

Soonchunhyang University Seoul Hoapital, Seoul, Korea

Methods

Medical records were reviewed of SS(n = 49 eyes), non-SS dry eye(n = 58 eyes), and non-dry eye patients(n = 59 eyes) underwent cataract surgery. Patients who followed for at least 12 months were enrolled. Uncorrected distance visual acuity (UDVA, LogMAR), ocular surface staining score (Oxford stain score, OSS), postoperative refraction, and mean absolute error (MAE) were evaluated for efficacy outcomes

Results

This study comprised 26 patients with SS, 30 patients with non-SS in Dry eye group, and 31 in control group. The OSS score was significantly higher in the Sjögren group at preoperative and each postoperative visit. (p <.001). There was no statistically significant difference in target refraction and MAEs of all postoperative visits among three groups measured with either IOL master or immersion ultrasound A-scan. Preoperative mean keratometry from two methods was strongly correlated in all groups (r >0.990, p <.001). UDVA in the Sjögren group was significantly lower than the control group after postoperative 3 months, but not different from the dry eye group.

Conclusions

With proper preoperative and postoperative treatment of superficial keratitis, refractive outcomes of SS patients were comparable to healthy or non-SS dry eye patients. UDVA was less improved related to chronic keratitis in the Sjögren group.
