Vivity Iol In Patients With Previous Corneal Refractive Surgery

Purpose

To present the refractive and functional results of two patients which present unilateral cortical and nuclear cataract and had previous corneal refractive surgery.In both cases was implanted an EDOF IOL- Acrysof IQ Vivity (model DFT 015).

Methods

We report two cases of unilateral cataract and corneal refractive surgery history, none of them documented. In both cases endothelial cell count and OCT was normal and RE was dominant. First case, 49 years old male with LASIK 18 years ago in OU. BCVA RE=0.9(-0.75/-1.00x94˚), BCVA LE=0.2(-7.50/-2.50x90˚), IOP OU=10mmHg. We used optical biometry and Barret True-K formula (AXL RE=27.38mm, LE=27.73mm) and implanted in LE: +18D Vivity IOL for -0.30sph, using FLACS. Second case, 54 years old male with PRK 10 years ago in OU. BCVA RE=0.4(-1.50/-0.50x70˚), BCVA LE=1.0(+0.50/-0.50x122˚), IOP OU=15mmHg. We used optical biometry and Barret True-K formula (AXL RE=25.41mm, LE=25.49mm) and implanted in RE: +21.5D Vivity IOL for -0.10sph using FLACS.

Results

Visual acuity was assessed at one day and one month after cataract surgery. First case, refractive refraction at LE was -0.50 -0.25x41˚, uncorrected decimal visual acuity LE (distance (5 m)/ intermediate (66 cm) and near (40 cm) was 0.7/0.8/0.63 and BCVA LE (distance (5 m)/ intermediate (66 cm) and near (40 cm) was 1.0/1.0/1.0.  In the second case,refractive refraction RE -0.25-0.25x117˚, uncorrected decimal visual acuity   RE (distance (5 m)/ intermediate (66 cm) and near (40 cm) was=0.7/0.8/0.63 with  and BCVA RE (distance (5 m)/ intermediate (66 cm) and near (40 cm)  was 1.0/1.0/1.0.  

Conclusions

 

  1. The implantation of the new EDOF AcrySof IQ Vivity, offers good visual acuity at all distances without correction and a vision without disturbances such as glare, rings around lights, starburst nor difficulty while driving at night. Patient satisfaction was high in both cases, showing a very good binocular vision.
  2. The Barret True- K formula is safe and predictable in patients that had undergone corneal refractive surgery.
