Precision Pulse Capsulotomy To Reduce The Risk Of Komplikations Of Advanced Cataracts

Purpose

To evaluate the safety and efficacy of  precise capsulotomy  device Zepto (Mynosys Cellular Devices) in advanced cataracts.

Methods

From January 2021- January 2022, 26 eyes of 26 patients  with advanced cataract were included in prospective study.The inclusion criteria were advanced posterior polar cataract,mature,white  and hard nuclei cataract (cataracta brunescens)Included were 6 eyes with advanced cataract with narrow pupil 

Excluded wereeys with pseudoexfoliation,insufficiens of zonula fibers and  lens dislocation.

Results

Precision Pulse Capsulotomy  created in all cases  a circular anterior capsulotomy of a precise diameter.In one case anterior capsula tear occured,even so the intracapsular  stable implantation of IOL was possible.The tear was caused  with sharp nucleus fragment.In no case posterior capsular rupture occured.BSS floats the Zepto tip off the capsule enter the subcapsular space in a 360 degree fluid wave that begins at the capsulotomy egde and separates LECs from the anterior capsule.This facilizate the phacoemulsification and the removal,aspiration of epinucleus and cortex and accelerates the whole procedure In 6 cases with narow pupil the iris extension device was used after the CR.

The achieved position of all IOL in capsular bag was stable.

Conclusions

Precision pulse capsulotomy is a safe effective procedure for the advanced cataract Zepto reduces the risk of capsula ruptur and facilizes removal  

of the cortex and epinucleus . 

It is as well convenient for cases with small pupil
