Intrastromal Corneal Ring Segment Implantation After Bowman Layer Inlay Transplantation – A Case Report.

Presentation of a case of an intracorneal ring segment (ICRS) implantation after a Bowman layer transplantation (BLT) procedure in a patient with keratoconus (KC), with good visual outcome.

Ophthalmology department of Centro Hospitalar Universitário do Porto (CHUP).

A 22 year-old patient with a 10 year follow-up history of progressing KC . A first attempt for a ICRS implantation when he was 13 years old was unsuccessful. He did not meet eligible criteria for collagen cross-linking and his young age raised concern about performing a penetrating keratoplasty/lamellar keratoplasty. He was submitted to a BLT with further ICRS implantation, achieving both disease stability and improvement of his visual acuity.

This case exhibits the importance of both the imaging exams during follow-up of patients with keratoconus in order to optimize their medical and surgical treatment, as well as the new surgical options and its potential benefit, when combined. The combination of both BLT and ICRS implantation has not been previously described in literature.
