Cataract Surgery With Wave Front Correcting Edof Intraocular Lens In Fuchs Endothelial Dystrophy

 To show the compatibility of implantation of  wavefront EDOF lens in a case report of Fuchs endothelial dystrophy without any functional hexagonal endothelial cells interested in presbyopic correction intraocular lens achieved intermediate and near distance vision with an acceptable quality of vision.

73-year-old man with myopia presented with advanced Fuchs endothelial dystrophy, refraction of -3.25diopters (D) with a cylinder of -1(at 102 degrees) and -3.50 D with a cylinder of -0.50 D (at 47 degrees)with no hexagonal cell/mm endothelial cell count in both eyes, respectively, and corrected distance visual acuity of 20/40 in each eye with moderate nuclear cataract in both eyes. The cataract surgery was done with a wavefront-correcting EDOF IOL (Vivity, Alcon, Texasintraocular lens (IOL) alone because of the advanced Fuchs endothelial dystrophy and high risk of corneal decompensation and reduced optical quality postoperatively. A successful cataract surgery was done with a hydrophobic EDOF IOL with a refraction target of -0.50 D. At 3 months, the uncorrected distance visual acuity was 20/25 in each eye, with uncorrected near visual acuity of 20/25 for 40 to 60 cm from the face. The endothelial cell count at 3 months was cells/mm in both eyes. Although the longer recovery of time for each eye after the surgery the patient gained both distance and near visual acuity with an acceptable quality of vision without any dysphotopic disturbances confirmed by the modulation transfer function of aberrometer.

Wavefront correcting EDOF IOLs might be a good indication in a patient with advanced Fuchs endothelial dystrophy interested in presbyopic correction after the cataract surgery even without lamellar transplants.

It is recommendable to define the final target of -0.50 for the IOL selection if in the future it is necessary to do any lamellar transplant.
