Post-Cataract Surgery Ulcerative Keratitis In A Patient Followed For Sjogren’S Syndrome

Purpose

We report the case of a 62-year-old patient followed for Sjogren's syndrome, and who presented postoperatively from cataract surgery with a preperforative ulcer.

Setting/Venue

Ophtalmology emergency department of 20 Aout 1953, university hospital, Casablanca

Methods

62-year-old patient whose preoperative examination did not report any particular pathological history.

The patient was operated on for her cataract in her right eye. The control after one week was satisfactory.

Two weeks later, she presented with a deep round paracentral ulcer and clean and ocular dryness in the  Adelphe eye.

Results

After checking her old medical documents, we found that she was followed for Gougero-SJogren, and we stopped NSAIDs and local corticosteroids which are replaced by general corticosteroids and macrolide associated with local artificial tears.

The evolution towards healing was very slow.

Conclusions

surgery may aggravate the underlying systemic pathophysiological processes associated with Sjogren's syndrome.

Nonsteroidal anti-inflammatory drugs (NSAIDs) and steroidal eye drops should not be used in patients with local or general risk factors for ocular surface damage unless absolutely necessary.
