Purpose

to show that patients treat residual refractive errors after Lasik and presbyopic problems with intrastromal fresh myopic lenticule implantation with SMILE.

Eye Hospital

Pristina,Kosovo

Methods

  48 eyes of 24 patients (38-55 years old) with a postop Lasik hypermetropic residual refraction
 The donor and recipient patients were notified and scheduled for Treatment on the same day. Before the donor patients underwent SMILE, the refraction was corrected by extracting the lenticule from a small incision of 2 mm using 6.6 mm lenticule diameter, 7.6 mm cap diameter with 130 μm cap thickness, and 130 nJ energy. The myopic lenticular implant was placed in BSS  before implantation 
 After, the recipient patient created a stromal pocket with a diameter of 7.60 mm and a cover thickness of 140 μm from the corneal surface and a 2 mm incision. The pocket was dissected using a blunt spatula. implanted fresh myopic lenticule
  

Results

The patients were followed for an average of 1 year.UDVA was increased from (preop 0.65±0.09 LogMAR to postop at 12 months (0.07±0.05 LogMAR; p<0.001).The patients' pupil diameters were 5.63 ± 0.86 [4.4 to 7.3] mm.UNVA (35cm) was increased that,preop from 7J [5 J to 8J] to 2 J[1J to 2J] 1-year postop, it was statistically significant (p<0.001).UNVA (70 cm) preop from 6 J[3J to 7]J to 3J [2 J to 4J] postop 1-year, this improvement was statistically significant (p<0.001). Patients reported satisfaction when reading a book, looking at the phone, and using a computer. No disturbance from the lights was observed while driving at night. All patients Binocular read more comfortably postop 100 words in a shorter time than preop.

Conclusions

We observed increases in patients' corneal spherical aberrations in the short term. In addition, we saw an increase in the negative Q value of the cornea; since we implanted an intrastromal fresh myopic lenticule, we think that it affects the presbyopic correction.The strengths of this method are that patients do not have accommodation problems, are economical, and start their daily lives in a short time. Postop Lasik residual hypermetropic refraction and presbyopia treatment with Smile are effective, safe alternative methods.
