Impact Of The Axial Length In Clinical Results After Implantation Of A Hydrophilic Toric Intraocular Lens

Purpose

The purpose of this study was to compare the clinical outcomes after implantation of a hydrophilic toric intraocular lens (tIOL) in eyes with an axial length (AL) of <25 mm versus >25 mm.

Setting

Prospective interventional two-arm single center study

Methods

A total of 50 eyes will be enrolled in this prospective, single center study. Until now, the Torica-aA (HumanOptics, Erlangen, Germany) was implanted in 23 eyes. Thereof, 18 eyes had an AL of <25 mm and 5 eyes an AL of >25 mm. The mean age was 67.3 ± 7.5 years. Preoperatively, the mean corneal astigmatism was 2.6 ± 1.1 Diopters (D) (Range 1.1 - 5.0 D). Postoperative evaluations were performed after 1 day, 1 week, 1 month and 4-6 months.

Results

One month after surgery, the mean refractive astigmatism was reduced from -2.6 ± 1.4 D to -0.89 ± 0.46 D (p < 0.001). The refractive astigmatism, one month after implantation, measured -0.94 ± 0.50 D and -0.67 ± 0.14 D in eyes with an AL of <25 mm and >25 mm, respectively (p = 0.185). The mean corrected visual acuity improved from 0.37 ± 0.21 logMAR before surgery to 0.07 ± 0.10 logMAR after one month (p < 0.001). The difference between the calculated and the actual toric axis was 3.79 degrees overall. The mean difference of the axis position, one month after implantation, was 4.38° and 0.67° in eyes with an AL of <25 mm and >25 mm, respectively (p = 0.159). No relevant complications occurred, neither intraoperatively nor postoperatively.

Conclusions

In this interim analysis, the Torica-aA showed a good clinical outcome and can be regarded as an appropriate toric intraocular lens for reducing corneal astigmatism in eyes with regular and long axial lengths. A final conclusion can be drawn after the full completion of the study.
