Purpose

Managing patients' expectations and satisfaction regarding visual acuity after cataract surgery. We aimed to investigate improved visual acuity and patient satisfaction after small incision lenticule extraction (SMILE) in pseudophakic (Trifocal IOL) patients with residual myopic refraction after cataract surgery.

Setting

Department of Ophthalmology, Eye Hospital, Prishtina, Kosova; 

Methods

Seventy-six patients (82 eyes) who underwent cataract surgery with ZEISS AT LISA tri 839MP IOL implantation were included in this retrospective study. Included patients were 56-79 years, wanted spectacle independence, and had preoperative myopic refraction between -1.0 and -2,25 diopter (D), and astigmatism between -0,75 and -1,75 D. Treatment status was defined as trifocal IOL (n=82). SMILE was performed in dissatisfied patients after cataract surgery, and the patients were followed up for one year on average. We evaluated the visual acuity and satisfaction and examined laser vision correction and level of satisfaction in patients dissatisfied after Trifocal IOL implantation.

Results

Possible reasons for the patients' dissatisfaction were reading books, using a computer, and driving at night.Visual acuity was improved After SMILE, the residual myopic refractive error (sphere) was decreased significantly pre-op  -2.08±0.28 [-2.25to -1.0] to -0.25±0.20 [-0.5 to 0] post-op 1 year; p<0.001). Visual acuity  was increased that, UDVA at 1 month (pre op. 0.65±0.08 [0.52 to 0.7] LogMAR  and postoperatively 0.09±0.02 [0.05 to 0.1] LogMAR ; p<0.001) and,at 6 month UDVA was increased  from 0.09±0.02 [0.05 to 0.1] LogMAR to 0.06±0.02 [0.05 to 0.1] LogMAR  at 12 months ; p<0.001),Patient satisfaction measures Post-op SMILE (reading, night driving, and using a computer) were significantly improved.

Conclusions

SMILE is one of the reliable methods in treating residual refraction after cataract surgery as it gives results in the shortest time without complications. This treatment also increases patient satisfaction and improves vision in a short time.Two important concepts in this study, it was seen that the path followed without increasing the Q value too much and reducing the Total RMS too much-brought success
