Modified Four Flanged Intrascleral Iol Fixation Technique In An Eye With Dropped Iol

To present a challenging case of dropped IOL (intraocular lens) in corpus vitreum 5 days after cataract surgery that is fixated with modified 4 flanged intrascleral fixation technique.

A 67-years-old Caucasian male presented to our clinic complaining of immediately reduction of vision in right eye after 5 days of cataract surgery in another clinic. There was no history of trauma. On examination best corrected visual acuity (BCVA) was 2/10 because of mild corneal edema, Descment membrane folds and vitreal dimness. Fundoscopy examination revealed dropped Bauch&Lomb acreos IOL in the corpus vitreum. At this point the patient was submitted to modified 4 flanged intrascleral fixation technique using 7:0 polypropylene and 30G neddle, considering 4 haptics of subluxated IOL as 4 flanges. No intraoperative complications were reported. One month post operatively, the patient present with BCVA of 9/10.
Intrascleral IOL fixation using the subluxated lens is a surgical challenge but it shortened the OR time and avoid corneal trauma during IOL exchange.
