Purpose
To systematically review the literature focusing on the clinical outcomes of the KeraKlear (KeraMed, USA), a foldable intrastromal keratoprosthesis.
Setting
Systematic review of the literature
Methods
We searched 6 databases using 4 key words, i.e, KeraKlear, Foldable Keratoprosthesis, Intrastromal Keratoprosthesis and Non-penetrating Keratoprosthesis. Included studies had to be conducted in vivo on humans, published until January 3, 2023 and had to investigate the implantation of the KeraKlear. We aimed at recording the post-operative complications, the rate of prosthesis retention and the mean improvements in visual acuity.
Results
We identified 144 publications, 6 of which, including 38 eyes, met the inclusion criteria. No randomized controlled trial was found. Post-operative complications occurred between 0% and 50% of eyes, whereas 21% of patients had an implant-extrusion or needed a reoperation. Postoperative visual acuity improved by a mean of -0.79±0.59 LogMAR and retention rates ranged between 50% and 100%.
Conclusions
Despite the limited quality and quantity of evidence, the available literature seems to suggest that the KeraKlear is a valuable tool in the treatment of complicated corneal disorders.
