Late diffuse lamellar keratitis after blunt trauma without flap damage

Abstract Details


To report a case of blunt trauma with ball and subsequent diffuse lamellar inflammation 6 years after LASIK procedure.


Birinci Eye Hospital, Istanbul


A 29-year-old man was referred after a blunt injury of his left eye due to a ball.He initially presented with pain, loss of vision, corneal edema,hyphema and no damage was noted on the flap structure. Intraocular pressure was 22 mmHg on the day of presentation. After 3 days of presentation, grade 4 diffuse lamellar keratitis (DLK) was developed.


Intensive topical steroid treatment was ineffective on DLK and interface irrigation was performed on the second day. The corneal edema and DLK was resolved following the irrigation. Mild flap folds were visible after the irrigation. Following up of 45 days after procedure the central corneal thickness was changed from 621 to 467 and vision improved from hand movement to 0.8.


Late DLK with corneal edema may develop after blunt trauma to eye withoth any flep dammage and interface irrigation can provide significant vision improvement.
