Superior keratoconus presenting with hydrops

To report the first case of superior keratoconus presenting with hydrops.


Review of patient chart at Eye Treatment Centre, Whipps Cross Hospital, Whipps Cross Road, London, E11 1NR.


A 48 year old African male patient was referred to our centre from his optician with poor vision and high astigmatism in both eyes. His vision was 6/19 OD with 1.50D x -7.50 x 92° and 6/38 OS with -1.75D x -5.00 x 70°. Slit lamp examination found left corneal scaring and resolving hydrops. Oculus pentacam tomography revealed bilateral marked superior steeping consistent with a diagnosis of superior kerataconus. The patient was managed with topical antibiotics and lubricants for the hydrops.


Superior keratoconus is a very rare manifestation of corneal ectasia, and to our best knowledge, this is the first case presenting with hydrops described in the literature.
