A young female presented with extreme xerophthalmia refractory to topical treatment with lubricants and autologous plasma formulations. On history taking she admitted to have undergone sequential and extensive bowel surgery for obesity. Vitamin A deficiency was suspected and the patient was referred to the internists for further investigations. She was found to have low levels of Vit A,D,E,K . She received intravenous supplementation and achieved rapid recovery.

As surgical treatment for obesity is becoming more popular both patients and surgeons should be aware of potential ocular risks of very extensive surgery and regular follow ups are needed in order to identify early signs of Vitamins depletion.
