The aim of our study was to evaluate clinical outcomes, rotational stability and patient satisfaction after the implantation of the trifocal diffractive toric IOL Bi-Flex 677MTY.

In this prospective, interventional study 10 patients with corneal astigmatism between 1.5D and 8.0D were implanted bilaterally with trifocal toric Bi-Flex 677MTY IOL (Medicontur). Patients were examined 1 and 7 days, 1, 3, 6 and 12 months postoperatively. Best distance-corrected and uncorrected visual acuities (VA) were tested at distance (4 m), intermediate (67 cm) and near (40 cm) using ETDRS charts. Photodocumentation was prepared at each visit to evaluate IOL position and potential rotation. Binocular defocus curves were obtained with the subject’s best distance correction in place. Contrast sensitivity was evaluated with CSV-1000 system. Patient satisfaction was also assessed.


The mean spherical equivalent and cylinder improved from 2.15D to 0.28D and from 3.51D to 0.57D, respectively. The mean monocular uncorrected distance, intermediate and near VA was 0.92, 0.80 and 0.78 in decimal, respectively. Intraoperative to 3 months postoperative comparison of IOL axis alignment showed low levels of rotation (mean 2.04±1.84°) and 0 degree thereafter. The binocular defocus curve at 6 months shows a wide range of useful vision with 0.8 ± 0.17 (decimal) at −1.50 diopter defocus.


After cataract surgery, the evaluated trifocal toric IOL successfully improved the visual functions of patients with pre-existing regular corneal astigmatism in all three evaluated distances with very good level of refractive precision and rotational stability.
